Each year stroke affects almost 800,000 Americans, with approximately 7% having some residual impairments. Following stroke, nursing and other interdisciplinary rehabilitation team members working together can dramatically improve the quality of life and functional capability of the stroke survivor. This presentation describes the state of the evidence pertaining to stroke rehabilitation and the 2010 published American Heart Association/American Stroke Association Guidelines.

1–5 pm The ABCs of the RAC Program: Impact on Inpatient Rehabilitation (002) 3.75 CH

This session will provide a basic foundation of the Recovery Audit Contractor (RAC) program and its impact on inpatient rehabilitation. Common reasons for denial and steps to the appeal process will be outlined. Clinical case scenarios will also be reviewed and discussed by attendees to determine whether reasonable and necessary care is present.

1–5 pm Medication Management Workshop: Optimizing Outcomes and Preventing Medication-Related Harm (003) 3.75 CH
Speaker: Demetra Antimisiaris, PharmD CGP FASCP
This workshop shares the essential techniques necessary to successfully evaluate and address the causes of medication-induced problems. The session will
- include an overview of medication-related harm (polypharmacy) versus effective multiple medication (polymedicine) use and medication-induced functional impairment
- discuss whether we are ready for the Baby Boomer generation
- highlight special challenges in rehabilitation medicine and long-term care
- share tools and methods to address medication management.
After each topic, small groups will solve case-based problems and present solutions. The cases will illustrate how basic principles together manage complex cases of polypharmacy and unintended medication-induced impairment.
